For NIC or Emily Carr Students:

Students must be enrolled, full time, in either the North Island College or Emily Carr program to qualify. Students may share a booth space with other students. Fee for a 12’ x 12’ booth space is $90 plus GST of 5% . If there will be more than one student in the booth sharing costs, please pool the fees and make only one payment of $90 +GST of 5%. (Eg. If there are 3 students fee would be $30 each plus GST of 5%. since only one payment of $90 +GST of 5% should be made)

I am applying to enter the Originals Only Fine Art Show & Sale held on Saturday and Sunday, August 12th & 13th 2017, at the Comox Marina Park, and will adhere to the conditions of entry. My booth fee of _________ ( single booth fee – $120 plus GST of $6 for a total of $126.00, double booth fee – $240.00 plus GST of 5% ($12.00) for a total of $252.00, (cheque payable to “The Town of Comox”) is submitted with this application; and will be refunded in whole if I am not able to be assigned a booth. I have read this document in its entirety and agree to the terms of the Originals Art Show Rules & Regulations. I understand that I may NOT display any reproductions in this show other than small greeting cards, and that I may be asked to leave without refund if I persist in doing so.

Originals Only Art Show and Sale Liability Waiver
August 12th & 13th 2017, – Marina Park, Comox

This waiver must be submitted along with your application: With my acceptance as a participant in The Originals Only Art Show and Sale or any activity associated with it, I agree to release The Town of Comox and the Organizing Committee of the Originals Only Shows and/or their employees, agents or volunteers from and against all claims, actions, costs, expenses and demands in respect of death, loss, injury or damage to my person or property whatsoever caused, arising out of or in connection with my taking part in the event and notwithstanding that the same may have been contributed to or occasioned by any act or failure to act ( including without limitation, negligence ) of members of The Originals Only Group, The Comox Recreation Commission, The Town of Comox and/or any one or more of their employees, agents or volunteers. This agreement is binding on me, my heirs, executors and administrators.
